1. Valid and Signed Official Passport Passport must have one blank visa page and be valid 6 months beyond stay.
- Amendment pages in the back of the passport are not suitable for visas

2. Photographs
Two recent passport type photos, in color, on photo paper, front view and with a white background - Size 2x2 No facial expression on the photo. Both the photo in the passport and the visa application must be different. The same photograph may not be used.

3. Visa Application Form
Must be fully completed. -On page 3 of the form, applicant must sign and date in two places. SECTION 37 and last SECTION of the application. - Applications not completed properly may cause a delay in the issuance of the visa - If a field is not applicable- put N/A or None Download Application here:

4. State Department - Cover Letter
