Two recent passport type photos, in color, on photo paper, front view and with a white background - Size 2x2 The applicant will need to upload their photo and signature, the photo uploaded must be the same photo provided |
3.Visa Application Form
Two(2) visa applications must be sent- (Original signature plus one copy) - Applications not completed properly may cause a delay in the issuance of the visa -Please do not leave any field blank and answer each question in detail - If a field is not applicable- put N/A or None Follow the link to complete the online form: BRAZIL VISA APPLICATION - Print the receipt and sign inside the box. 4. Travel Itinerary
Itinerary from airline or travel agency indicating the following - Applicants Name - Dates of travel(to and from country) It is recommended that tickets are not purchase or confirm any tickets until the visa has been issued. |
5. Proof of Jurisdiction
Options to satisfy this requirement: 1. Clear copy of driver's license or state identification OR 2. Utility Bill with applicants name and address on it. (telephone, cable, or cell phone bill NOT acceptable) OR 3. Lease agreement signed by both landlord and applicant Address on proof of jurisdiction must match address on application. |
6.Yellow Fever Vaccination
Only required if applicant recently visited the following countries within the past 90 days. Angola, Benin, Bissau Guinea, Bolivia, Burkina Faso, Burundi, Cape Verde, Cameroon, Central African Republic, Colombia, Congo, Democratic Republic of Congo, Ecuador, Equatorial Guinea, Ethiopia, French Guiana, Gabon, Gambia, Ghana, Guiana, Guinea, Ivory Coast, Kenya, Liberia, Mali, Mauritania, Niger, Nigeria, Panama, Peru, Rwanda, Sao Tome & Principe, Senegal, Sierra Leone,Somalia, Sudan, Suriname, Tanzania, Togo, Trinidad and Tobago, Uganda or Venezuela. |

4. Business Letter- US Entity- ORIGINAL REQUIRED
Letter from US Company or Organization- the signature must be original and the letter must be on company letterhead. - Nature of Business to be performed -Name and Address of your reference that will be visited -Dates of intended visit -Guarantee of return transportation and financial guarantee for the visit -IT IS EXTREMLY IMPORTANT THAT THE LETTER INCLUDES "no technical work or assistance" will be provided during stay in Brazil" - Signed by an officer or manager(not to be signed by visa applicant) Download Sample:

5. Invitation Letter
A letter of invitation from sponsor (either host company or individual) explaining the relationship to the applicant and the purpose of travel. The letter must indicate the length of stay, place of stay, and contact information. - Letter must be addressed to the Consulate General of Brazil
