1. Valid and Signed Official Passport
Passport must have one blank visa page and be valid 6 months beyond stay. - Amendment pages in the back of the passport are not suitable for visas

2. Photographs
Two recent passport type photos, in color, on photo paper, front view and with a white background - Size 2x2

3. Visa Application Form
Must be fully completed. -Must be typed and all fields must be filled out. WE NEED: -2 ORIGINAL SIGNED APPLICATIONS, PLUS 1 COPY -2 COLOR COPIES OF THE PASSPORT(PASSPORT MUST BE CENTERED ON PAGE) - Applications not completed properly may cause a delay in the issuance of the visa - If a field is not applicable- put N/A or None Full name: List the organization name and the name of your point of contact in the UAE or US Embassy Profession- List the industry Address: List the organization's address in the UAE Telephone- list the phone number for your point of contact in the UAE Sponsor Information- Must also state US Embassy with full address and contact details Download Application here:

4. State Department - Cover Letter
Name, Title, Purpose( nature of visit and contact in country), and Dates of Travel
Must Include: >Request for the visa- "The organization/agency requests a diplomatic or official visa for.." >Reason for the visa Name, address, and phone number of your point of contact within the organization in the UAE or US Embassy in UAE |
5. Letter from sending department or Agency
Name, Title, Purpose( nature of visit and contact in country), and Dates of

1. Gather your passport,
documents, and send them to us via UPS, DHL, or FEDEX. (We recommend a courier that allows you to track the package). 2. Once we receive your package, we will verify the documents and submit them to the appropriate Consulate.
