1. Valid and Signed Official Passport Passport must have one blank visa page and be valid 6 months beyond stay.
- Amendment pages in the back of the passport are not suitable for visas

2. Photographs
Two recent passport type photos, in color, on photo paper, front view and with a white background - Size 2x2

3. Visa Application Form
Two(2) visa applications must be sent - Applications not completed properly may cause a delay in the issuance of the visa -Please do not leave any field blank and answer each question in detail - If a field is not applicable- put N/A or None Download Application here:

4. State Department - Cover Letter
Name, Title, Purpose( nature of visit and contact in country), and Dates of Travel

5. Letter from sending department or Agency

7. Yellow Fever Vaccination
Yellow fever certificate required. Clear copy of all pages. |

NEXT STEPS..........
1. Gather your passport,
documents, and send them to us via UPS, DHL, or FEDEX. (We recommend a courier that allows you to track the package). 2. Once we receive your package, we will verify the documents and submit them to the appropriate Consulate. (Should we need any additional clarification or documents, you can be sure that we will contact you). | |||
