1. US Passport

2. Photograph
One recent passport type photo, in color, on photo paper, front view and with a white background - Size 2x2

3. Passport Application Form- passport will be sent to the "mailing address" indicated on form.
DS-82 - Follow the link and complete your application online. It will generate a pdf application, print 2 copies https://pptform.state.gov/?Submit2=Complete+Online+%26+Print ***Renewing a 2nd passport- Reference the passport that is being renewed ***Renewing a 2nd passport- send us a color copy of your 10 year primary passport ****Requesting a brand new 2nd passport, reference the Primary Passport Please make sure that you use a high quality quality printer and the resolution on the printer is of good quality |
4. Travel Itinerary
optional-not mandatory Itinerary from your airline or travel agency indicating the following - Applicants Name - Dates of travel(to and from country) OR Letter from your employer on letterhead and with original signature stating the nature of the trip, travel dates, and your information. |
5. Letter of Authorization(LOA)
Please print the LOA and sign it with blue ink. This letter is an authorization in which you authorize us to process your passport.

6. Identification
Front and Back copy of your government issued identification. |
7. Second Passport Request Letter
Through this letter you will request for the second passport. Please be specific and make sure that you sign the letter yourself. Please use the sample as a guide and print it on your company letterhead. -The BOLD text on the on the sample letter is mandatory. Make sure to tailor the letter to your specific request.

8. Passport Work Order
The work order is our internal form that includes your basic info as well as your travel and shipping specifics This form can be handwritten. Download Form here:

2nd passports are processed first, the primary passport will be returned about 2.5 weeks later
**We are only able to submit Tues-Wed-Thurs and our work goes out the night before. Please take this in consideration when applying for a 2nd passport

- When you download the "Passport Work Order," there will be a space to include your payment information. We accept VISA, MASTER CARD, AMEX, and US Postal Money Order as form of payment. We will send you an itemized invoice along with your passport.

Once we receive your package, we will hand carry the package to the Washington Passport Agency (Should we need additional clarification or documents, you can be sure that we will contact you).
